During your first visit, we will do a comprehensive, full-mouth examination to formulate a plan for how to best serve your dental needs. Dr. Vikas Puri will present specific recommendations for maximizing the health of your mouth and therefore also improving your systemic health. We will provide you with the roadmap to health and schedule your next appointments to begin your journey.
To expedite our first visit, please complete the enclosed patient information and health history form and return it in the enclosed envelope. Please include any dental insurance, as we are experts at working with any and all companies to maximize your insurance benefits. We will provide you with detailed financial estimates at your initial visit as well.
Please take premedication as directed if you are required to do so before dental appointments.
